I think it varies depending on which office you are at. The one I worked for started fairly low but had an extensive tiered structure. Probably not much different in the end from most average companies. You really should check with the individual office you are applying with, as I suspect there are vast differences between them all. MD-IT was gobbling up little MTSOs from all over the country and so no doubt the rates are quite variable.
The most difficult thing about making money, though, was the platform. The sound quality was not so good and you have to run Express Scribe along with it, and go back and forth to get and release jobs. But most frustrating was having to use the mouse for everything - just really slows down production!
