I really do think some MTSO's are missing this - base cost of transcription rate to rate of dictation difficulty, and pay premium to MT. Just one really lousy dictator can make the difference between a fair day's production and pay, and a mediocre or poor one...

about not updating to internet connection instead of using C-phones.  I guess they do it because they find people who will accept it.   It also seems odd to me that they are willing to risk the reputation of an at least decent company to keep one account that is ruining their image.

At the same time, what can we do to get physicians and the health care system to recognize this?

For all their education, docs do not seem to understand that it's unproductive to pay two people poorly rather than pay one person well. It's totally disrespectful of our field.

The problem is, it seems like a lot of transcriptionists manage to perform their job in a somewhat rote fashion, recognizing terminology, but not understanding the subject matter, or even language for that matter.

If we don't weed out these transcriptionists, it lowers the bar for all of us.


This is an excellent point and the root of alot of our problems.

I have been thinking about this very same thing.  Some companies want the experienced MTs with decades of experience so they do not have to offshore or pay QA to re-do reports.  There are only so many of those jobs out there.  Other companies want to get rid of us because we may too much money.  They can offshore or have QA clean up the mess a newbie makes. 


Then you can go middle-of-the-road, if you will.  SOOOOO very many MTs are out there willing to take what we refuse.  Where does that leave us?

I work for CardioScribes ... I just am not a cheerleader.  I see their warts, same as I see the warts on other companies.  I'm sure they see my warts as well. 
I don't work in the office and never claimed to. I am just saying that since the new office open
October with everyone of the admin staff in one place, these problems do not occur any more.
I understand your point, my point is why can't MTs and QA work together..
being QA doesn't mean you are above everyone else, you are just an extra set of ears...and why do so many MTs take offense to QA??? That is my point...you made have had bad experiences in the past with QA but not all QA are bad...

So I am curious, what company do you work with to make 950-1000 a week? I will be a new MT at the end of May next year, and I would like to know. If you dont mind that is.

I meant that is what it would cost to get a good family policy without a ridiculous deductible through other companies.

I was offered a position by another company and turned it down because the premium was $900+ per month. However, it covered a lot more and had much less out of pocket than this policy has turned out to have. Maybe I should have chosen differently.

TA Speed sm
I don't use TA Speed, I use Instant Text, so I can't answer your question about imports into it. However, I do use AutoCorrect for correcting my typos, and I have no problem with it. Your original AutoCorrect file will still be there, even after you install your software. I don't see why you couldn't just continue to just use your AutoCorrect if you wanted. During my initial training, I told the techie-type critter that I wanted to use my Instant Text (and not the TA Speed), and I *think* he went into the system and did some kind of techie voodoo, so keep that in mind (and mention it to the tech if/when you do work for them). =)
It took me a while to get up to speed
I do acute care only. I know when I was hired, I was told I would not be paid for any carbon copy information, but I don't remember about demographics...Demo information would only be a line or so, anyway at most.

I do know the computer strips out any hanging spaces at the end of paragraphs, but a lot of companies can do that with their platforms as well.

If your headers and footers are embedded in the template, I don't see how you could be paid for that as it would not be something you produced as part of the report.

As far as lines per pay period, I print out a copy of my schedule, jot down my line cout for the day and total it out at the end of the pay period. When I get my check, it always matches what I have calculated.

If you are limited in what type of work you can produce, you probably will have a hard time making lines. The more diversified you are, the more likely you can be cross trained and placed on backup accounts.

Reading this board, I am being disabused of the notion that I am any good at MT at all.  Been at it about 15 years and I'm a part-timer; always have been.  I came to it when I was over 45, as a retirement job.  


I don't get to use a lot of templates or normals, so this is not a factor.  On a good day, with really 'clean' dictations (good sound quality, clear dictator who is not constantly correcting himself) not having to spend a lot of time looking stuff up and with the wind at my back, I can produce maybe 250 lph for 2-3 hours.  I always thought this was pretty good.  Then I get fatigued and drop to 200 or so.  With really bad dictators/sound/subject matter, I get as low as 150.  I believe my knowledge and accuracy are good, but obviously my speed sucks. 


On this board I am seeing claims of sustained speeds of 300-400 lph or more.  So I'm thinking that maybe VR could actually IMPROVE my production. And as my company seems to be phasing this in ,account by account, I will soon have the opportunity to see for myself.   


I would like to take an informal survey of speeds and VR vs straight transcription.  Anybody willing to weigh in on this subject?

Been on VR for a year, been an MT for 25, have also been a book editor/manuscript proofreader. None of that matters one bit. And, no, VR does not learn and improve--at least not the platform I use. My top VR so far is 350 lph, average about 200-250. Main problem is reports with no punctuation, SR guessing at words, having to insert numbering, but my absolute favorite is docs taking personal calls while microphone is still on. I've heard everything from 2 docs discussing whether to seduce a woman with wine, candy, flowers, a skiing trip, etc, and then one of them calling in midst of same report to ask some chick if she'd sleep with him that night, that he was having dinner catered, also docs talking to their stockbrokers and car dealers, wondering whether to buy the green or the black Jag, and docs screaming at their kids and/or spouses--all this goes into SR and the old SR gremlin tries to turn it into part of the report, just as it does when the doc says--Go back and insert so and so under the diagnoses and take out so and so from the med list and add 3 more cc's, ad nauseum. All that stuff and more will slow you down terribly. You can never count on getting good clean SR reports for a whole shift. I think I need to get a job selling SR/VR systems but I couldn't stomach that kind of dishonesty to promise that MTs would be doing 600 lph in no time.

How many words are on a 65 character line - in general?


$100 per year? I was spending about $1000 sm
with my yearly membership to AAMT, a subscription to the journal, membership dues for my local chapter and fees for attending symposia for credits, the cost of driving/traveling to these things, etc. The math didn't work for me one single bit.
I produce around 1000 lines per day...
I have seen my production increase quite a lot in the last four months. When I first started I could only get about 500 lines a day...so I get a little more every day. :-)
I make 950-1000 per week

I am an employee, only 8 cpl with incentive over 1200 lpd, work a strict 8 hr/day, 40 hr/wk shift.  I do work on only one account, about 75% op notes, although it is a very large teaching hospital so still have a very large variety of dictators (currently wrestling with my list of 500+ new residents for this summery).  I have full hospital benefits.   It is possible.....


$700 is very good, except as IC you are losing 7.5% in having to pay your own taxes.      


Do you have enough work? 1000 lines a day?
I have read on some of the threads that there is not enough work to keep everyone busy.

I should put aside about $200.00 in the bank. I pay state tax, SS and federal, of course. That does not seem like that much to pay but I guess tax folks should know?
Over 1000?! I left there 18 months ago...
..and there were only 300 or so! They hired that many since then?

Q: What is there about your business or your business strategy that resulted in a Top 10 performance?


A: Mostly fundamentals. But two major initiates we took three years ago had an impact on our margins. We implemented speech recognition technology and last year started seeing the results of that technology. Twenty-five percent of our volume goes through the speech recognition engine and that has increased our margins and efficiency.


We also began, a year and a half ago, sending work overseas – to India – for overnight and weekend shifts, and that has had an impact on our gross margins.



Q: One source of strength for many companies hurt by the domestic economy has been the continued growth of foreign economies. How much of your success was due to foreign sales or investments?


A: We have transcribers in India who handle about 15 percent of the business. But our customers are in the continental United States.


The transcription industry in this country has inadequate staffing or capacity. We have a quickly aging population and as a result we can't handle all the required work. Out of necessity as well as for efficiency [we outsourced to India].



Q: Even if the economy does not sink into a technical recession, analysts see slow growth and weakness ahead for an extended period. What is your forecast, or guidance, for the economy and especially the sectors that most affect your business?


A: We don't give guidance as such but we tell our investors we would expect to grow the company by 20 to 25 percent on the revenue line. We can grow the bottom line at a faster rate than that because of improving margins. We will also contemplate growing the bottom line faster through acquisitions.



Q: What's on the horizon for your company that you can talk about, such as an acquisition, stock offering or buyback, new products, expansion.


A: We are right in the middle of the medical crisis. The electronic record is coming fast now — an electronic record that is available to the patient and the doctor. We [handle] 30 to 40 percent of that record automatically. One of the big cornerstones in the industry is the more efficient use of information, which reduces the redundancy of tests and other inefficiencies in health care that are related to turnaround time in hospitals. The patient is discharged from the hospital and that discharge summary may become the admission summary next time around, or maybe to a nursing home where the information is relevant.



Q: Will the computer eventually take over the job of the transcriber?


A: A big share will be done by computer, but we still have language and speech that will always be a requirement of the medical specialty to edit. The computer can help the transcriber do more. And this is a good career opportunity. These people work from home. They are in total control of their work.


 We have 1,000 domestic transcribers and 600 in India.
