What I would do.

I think I understand your plight somewhat. You may be hesitant or scared of confronting them due to the unknown of what the results will be, as we are not a society that values honesty in communication. I've been there and done that.
It doesn't seem like your current employer values you much if they treat you like this. How have you managed to stay with them this long; I think you should be rewarded for your tenacity and loyalty regardless of the fact that it is not reciprocated. Unfortunately, you do not have the time or energy to spend playing their game. This is YOUR life, and you are allowed to make the rules for your own game.
You may not feel comfortable confronting them directly. So, given that they have reduced your work access for 3 months now, it is my opinion that it may be in your best interest to seek other employment. Even if they stop playing this current game, it is likely that it could start up again. They have demonstrated to you how they operate; it is not likely that they will change.
Use the situation to your advantage and use this time to apply for other positions. Once you find another job, start the other job while you try to continue with the 500 lines per day at your current company.
Hopefully, the new job will work out fine, but while you are learning it, continue with the 500 per day at your current job. Once the other job is set, resign your current job.
When applying for your new job, you can ask them to not contact your current employer, or don't provide the name of your current employer if not asked.
The other possibility to consider is that someone in the QA department started this for some unknown reason and the owner/manager has no idea what's going on. If you really love the job, once you have a backup job lined up, you won't lose anything by speaking with the owner/manager. It is amazing to me what lengths some people will go to once they are given power over others' lives. I have met up with some of these types disguised as QA or QC, working at MTSOs.
Sometimes, as stated in other posts of this thread, the problem began at the top, trickling down from the client facility, from some equally dysfunctional manager working at the facility. Since they are unable or unwilling to communicate honestly and ask for rate decreases right up front, they choose to be indirect and complain about the quality of reports and try to justify rate decreases or gain some other benefit by claiming that the reports are of low quality. Just know that it is likely about them, not about you. Ironically, they usually are suffering from low self-esteem or simple inability to communicate, making your QA/QC process like that of the bullying that takes place with children. However, you do not have to subject yourself to this type of abuse simply because someone in the chain never learned how to communicate assertively and honestly.
