Here's what I did.

And it was about the same time of the year 5 years ago. I sent out my resume to nationals and got such a pathetic cpl quoted when and if they got back to me. SO I got some baskets at the GoodWill and spray painted them, got bags of candy at the dollar stores and wrote a business letter. Tucked it in the baskets and delivered them first to the closest clinics from home and then circled out some every day. 10 baskets got me 2 accounts at 12 cents per line. Put together a short agreement, set up delivery times and stipulations of their letterhead and envelopes, got their samples, made my templates and the rest is history. They are tape accounts and I already had my transcriber (but about $90 used here or EBay). Been happy ever since. Each clinic has added doctors/physical therapists and busier now than I really want to be - to tell ya the truth. I log everything for taxes, mileage log in my car for pickup and delivery, ink cartridges, etc, for taxes.
If anyone decides to try this, chiropractors, physical therapist, oral surgeons, podiatrists and more all need records transcribed. I recently added a backup on a flash drives for each office so they have their records on file with me (just in case something gets misfiled, I can reprint) and I have room on my computer and everyone seems to like that too.
Best of luck to you.
