If that's the case then why a form for each? Unless I'm mistaken, a criminal background check is pretty much public information at the county courthouse. Your insurance company can run updates to check for speeding tickets or any other traffic violations, which is why you may as well offer the information. I ran a credit report on myself just to see if there was any erroneous info on there and it was very time consuming and required a lot of personal information just to pull it up. Someone please correct me if I'm wrong.
