I work for a reasonably well-respected company with a reasonably good reputation for quality work.

The platform we use allows us to view previous reports and what an eye opener THAT experience was.  I take a lot of pride in editing to produce an accurate document that looks exactly like a transcribed document.  I even forsake getting a higher line count in order to produce a higher-quality document.

The tremendous amount of glaring errors I see in the prior reports just astounds me.  I was actually humiliated for my own employer at the quality of some of these documents I've seen while trying to double-check a medication or a prior procedure or whatever reason it was that took me to the prior reports.

I'm sure these are edited reports and not transcribed.  Nothing against editing as I personally greatly enjoy editing; it's just that it's painfully obvoius these documents have been put on maximum speed and just zipped right on through with as little correction done as possible.

I'm sure my line counts look low compared to those of some of my co-workers on this account who are obviously just skimming and uploading these documents as quickly as possible trying to make themselves a few extra bucks.

I don't even know what responses I'm looking for from my fellow MTs here. I guess I just felt the need to express my shock and horror of what is being put through as a finished document into a patient's medical record.

I think I'd rather have a little lower line count and not have another MT appalled looking at my documents.  Some of these are a downright embarrassment to the MT profession.
